Records desk / Missouri certified directory, counts only
Missouri certified home health agencies by metro county served, 2026
39 agencies in the Missouri Department of Health and Senior Services certified home health directory name at least one St. Louis metro county as a service area. St. Charles County is named by 35 of them, more than any other county. Lincoln County is named by 20, the fewest. This page ranks the seven Missouri metro counties on that count and publishes nothing else from the file, for a reason set out under the list.
The directory lists agencies the state has licensed or certified as home health agencies under Chapter 197 RSMo: clinical care at home under a clinician's plan of treatment, with skilled nursing among the services offered. A count here is the number of those agencies that name the county as somewhere they serve.
The directory holds no provider of non-medical care at home, because Missouri does not license or certify that category, and it holds no Illinois agency, because it is a Missouri file. The eight Illinois counties of the metro are absent from this page by construction, not because nobody serves them. And a named service area is a stated intention. It is not a promise that an agency has capacity in that county this month.
Methodology
- Metric
- Number of distinct agencies in the directory extract whose listed service area includes the county. An agency serving several counties is counted once in each, so the seven counts sum to 205, not 39. The share column divides the count by 39 and is the only derived figure on the page.
- Item set
- The seven Missouri county equivalents of the St. Louis metropolitan statistical area: the city of St. Louis and St. Louis, St. Charles, Jefferson, Franklin, Lincoln and Warren counties. The set was fixed by the metro definition before any count was read.
- Source
- Missouri DHSS, Home Health In-State Certified Providers 3dcz-79am on data.mo.gov, the same records the department serves through its provider directory. Portal record updated 2026-09-04.
- Pull date
- 2026-09-10. Extract: 39 agencies naming at least one of the seven counties. All 39 carry a Missouri address.
- What is withheld, and why
- Agency names, licence numbers, cities and expiry dates at row level. The data.mo.gov record for this dataset carries a null licence field, so its reuse terms cannot be read by machine and have not been confirmed by a person. Until the operator of this desk reads the portal's terms and confirms republication is permitted, this desk publishes counts from the file and not rows. If that confirmation never comes, the counts stay and the rows stay out.
- Sort
- Count descending, then county name. Jefferson County and St. Louis city tie at 32 and are ordered by name.
- Licence
- Null on the portal record. Operator confirmation required. Published as counts only.

How widely the 39 cast their net
15 of the 39 agencies name all seven counties. The rest narrow it, and two name only one. This second table is a distribution, not a ranking, and the ItemList on this page counts the seven counties above, not these buckets.
| Counties named | Agencies |
|---|---|
| all seven | 15 |
| 6 counties | 3 |
| 5 counties | 9 |
| 4 counties | 6 |
| 3 counties | 3 |
| 2 counties | 1 |
| one county | 2 |
7 buckets, 39 agencies. Same source and pull date.
Two further counts from the same rows. 25 of the 39 licences carry an expiry date in 2027 and 14 carry one in 2026, which means a third of the file is due for renewal within months of the pull date. An expiry date passing is a reason to check the directory, not a conclusion; renewals and file updates do not always land on the same day.
Why the rows are not here
This is the one file on the desk that names agencies and is not reproduced by name. The facts in it are public and published by a state department. The portal record that carries them states no licence, and this desk will not republish a state licensing file at row level, naming licensees with their licence numbers, on the strength of an assumption about terms that nobody has read. That is a records decision, not a data decision, and it is recorded on the sources page beside the Illinois file held for the same reason.
Nothing is lost to a family by the hold. The directory is free, searchable and current at health.mo.gov, and step two of the five lookups walks through searching it for one named agency, which is the only use a family has for it. A list of 39 names on this page would add nothing to that search except the appearance that this desk had vetted them. It has not.
New Plan Care is not in the Missouri directory, and should not be
The directory counted above lists Chapter 197 home health agencies. New Plan Care is not one and has never been one. It provides non-medical care at home in the Missouri towns it lists on its own site: personal care, companionship, homemaking, transportation, and support for a person living with dementia. Missouri does not license or certify that category, so a search of the directory for its name returns nothing, and that is the correct result rather than a warning. Do not read a licence, a certification or a state approval into this box. There is none to read, for it or for any provider in its category.
New Plan Care is a client of OwnersFirm, the company that publishes this site, and it pays for this box. The payment affected no count above, and the box was not earned by any figure on the page.
Call New Plan Care at (314) 405-0887.
