A shift schedule changes. Visits move, staff rotate, and a family member's own plans shift. The person who holds the calendar decides whether those changes reach the agency in writing and whether the household knows what was agreed.
Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.
Name the owner
Write one family member's name as the calendar keeper. Name a backup for the days the keeper is away. Give the agency both names and ask it to confirm them on its side.
Set the change route
Every change should go through a written message, not a passing remark at the door. Ask the agency to confirm each change in writing on the same day it is requested. If no confirmation arrives, follow up before the next scheduled visit.
Keep the log
Editorial: A plain log has four columns: the date of the change, what changed, who asked for it, and who confirmed it. Keep the log with the check sheet. The log shows what was agreed and when, which is the first thing to read if a visit does not happen as planned.
Review the calendar at each check-in
At each review, compare the calendar with the care plan and the log. Mark any visit that was moved without a written record. Bring the marked items to the agency's supervisor and ask for a written answer.
Where the lookups fit
The calendar does not replace the public files. If an agency's registration status changes, the calendar keeper should see the change in the lookup results. Use the city listing of federal in-home registrations to recheck the agency's name and address when the file is due for review.
