1,162 organisations in the fifteen county St. Louis metro hold a federal provider identifier under the taxonomy 253Z00000X, In Home Supportive Care. Folded from 65 cities into counties, 1,111 of them sit on the Missouri side and 51 on the Illinois side, and 3 of the fifteen metro counties return no registration at all. Counts only. No provider is named here, and the reason is in the box below.
A national provider identifier is issued to any organisation that asks for one and fills in the form. The record proves that an organisation requested a number, supplied an address and chose this taxonomy for itself.
It proves nothing about quality, licensure, accreditation, inspection, insurance, training or background checks, for any organisation counted here. No one verifies the work, no one visits, and the number is not withdrawn when a business closes, so a county total includes dormant registrations and related entities registered more than once. A county figure on this page is a stack of paperwork sorted by county. A tall stack is not a finding about care, and a short one is not a finding about its absence.

- The county. The 65 city rows already published on the city file, added up by the county that file names for each city. Nothing was re-queried.
- Metric
- registeredInHomeCareOrganizations, summed: distinct organisational NPI records whose practice location is in a city in the county and whose taxonomy list contains 253Z00000X. The second column counts how many of the county's cities returned at least one registration.
- Source
- NPPES NPI Registry public API, enumeration type NPI-2, queried once per metro zip code, paged to exhaustion, deduplicated by NPI, and filtered to records whose own practice-location zip is a metro zip. Spelling variants of Saint were merged at the city stage.
- The flaw in the county column
- The registry records the city an organisation typed, and the postal service applies the mailing city Saint Louis to the city of St. Louis and to much of St. Louis County alike. The city file assigns that one row to St. Louis city, and all 612 of its registrations travel with it into the first row below. This roll-up does not separate the city from the county for that address. It inherits the ambiguity and prints it. The honest reading of the first two rows is a single urban total of 948 that the file cannot split.
- Counties with nothing returned
- Bond County, Calhoun County and Jersey County returned no registration in any of their cities. They are listed under the table, unranked, rather than printed as a row of zero.
- Pull date
- 2026-09-10. NPPES changes daily, so a repeat of the query will not return the same total.
- What is withheld
- Provider names, NPI numbers and addresses, on every row and without exception. This page ranks counties. A list of organisation names under a heading like this one would read as a vetted list, and the source vets nothing.
- Sort

No registration was returned for Bond County, Calhoun County or Jersey County, all in Illinois. They are not ranked, and a zero here means no city in the county returned a record under this taxonomy on the pull date, nothing more.
What the county view adds, and what it repeats
The split across the river is the one thing this roll-up shows that the city table does not put in a single line: 1,111 registrations in Missouri against 51 in Illinois, with Madison County the largest Illinois county at 23. This desk does not know why the Illinois count is low and will not guess. Whether an organisation applies for an identifier depends on how it bills, not on whether it exists, and Illinois is also the side of the metro where this category holds a state licence, so the state directory there is the fuller record and the federal count the thinner one. In Missouri the position is reversed: the state publishes no directory of this category, and the federal count, for all its faults, is the only public measure of its size.
What the roll-up repeats is the mailing-city problem, and it is stated in the methodology rather than smoothed over. The 612 registrations under Saint Louis cannot be divided between the city and the county by anything in the file. Read the first two rows together, as 948, and read the rest of the table as the part of the metro the file can actually place.
