Every Medicare-certified home health agency with a street address in the fifteen county St. Louis metro that carries a CMS quality of patient care star rating in the July 2026 release. 47 agencies qualify. This is a copy of a federal file, sorted, not a review.
Everything below is a home health agency: an organisation certified by Medicare to deliver skilled clinical care at home, such as nursing and physical therapy, under a physician's plan of treatment. Medicare certifies these agencies and scores them, which is why they have stars.
Nothing below is a provider of non-medical care at home, meaning personal care, companionship, homemaking, transportation and dementia support. Medicare does not certify that category and CMS has never scored it, so no such provider can appear in this table and none is missing from it. Methodology
- Metric
- Quality of patient care star rating, as published by CMS. A single value from 1.0 to 5.0 in half-star steps, computed by CMS from a set of quality measures drawn from agency assessment and claims data. This desk does not compute it, adjust it or weight it.
- Second column
- HHCAHPS survey summary star rating, joined on the CMS certification number from a separate file. 38 of the 47 agencies have one. It comes from interviews with patients after care and is reported alongside, never blended in.

- Quality star descending, then survey star descending, then agency name. Ties are not broken by anything editorial, and no agency paid for a position. Positions are not for sale.

How to read the two columns
The quality star and the survey star measure different things and often disagree, which is informative rather than contradictory. The quality star summarises clinical process and outcome measures. The survey star summarises what patients said about the experience. An agency can run a tight clinical operation that people find impersonal, or the reverse. Both are averages across an agency's Medicare patients over a reporting period, and neither predicts what will happen in one household.
Three limits are worth stating. Agencies with too few cases in a measure are not scored on it, and 3 metro agencies have no quality star at all in this release, usually a volume question rather than a quality one. A star covers only the agency's Medicare patients, not everyone it serves. And the reporting period sits behind the publication date, so a star describes a window that has already closed.
The corresponding official tool, with the same underlying data plus measure-level detail, is Medicare's Care Compare. Anything here should match it. If it does not, the source is right and this desk is stale.
