Medicare home health patient survey stars, St. Louis metro, 2026

38 of the 47 star-rated Medicare-certified home health agencies in the fifteen county St. Louis metro also carry a patient survey star in the CMS July 2026 release, and 14 of the 38 hold the top mark of 5. The other 9 agencies have no survey star at all. They are named below the table rather than scored zero, and that gap is the first thing this file teaches.

Read this before the table: what category this file covers

Every row is a Medicare-certified home health agency: skilled clinical care at home, such as nursing and therapy, delivered under a plan of treatment ordered by a clinician. CMS surveys the patients of those agencies after care and publishes the result as a star. That is the only reason a survey star exists for any organisation in this table.

No row is a provider of non-medical care at home. Personal care, companionship, homemaking, transportation and support for a person living with dementia are not a Medicare-certified category, CMS runs no patient survey for them, and there is no star to copy. Methodology

Metric
HHCAHPS survey summary star rating, as published by CMS: a whole star from 1 to 5 computed by CMS from the Home Health Care CAHPS survey returned by an agency's patients. In this release every metro value is a 2, 3, 4 or 5; no metro agency holds a 1. This desk copies the value and does not compute, weight, round or adjust it.
Companion column
The quality of patient care star from the CMS provider file, printed beside the survey star so the two can be read against each other. It enters the sort only as a tie-break between agencies holding the same survey star, and it is the column the star ratings file sorts on instead.

Where the two stars agree, and where they part

Across the 38 surveyed agencies the survey star sits above the quality star for 23, below it for 10, and level with it for 5. The distribution of survey stars is top-heavy: 14 agencies at 5, 13 at 4, 9 at 3 and 2 at 2. 28 of the 38 are addressed in Missouri and 10 in Illinois.

The widest disagreements run in both directions. Jersey County Health Dept of Jerseyville, IL and Pike County Health Dept/home Health & Hospice of Bowling Green, MO each hold a survey star of 5.0 against a quality star of 2.0, 3 stars apart. Elara Caring of Bridgeton, MO holds a survey star of 2.0 against a quality star of 4.5, 2.5 stars apart the other way. This desk draws no conclusion from any of them. It prints them because a family reading one column alone would not know the other existed, and because the two columns are built from different things: one from clinical measures in assessment and claims data, the other from what patients said on a questionnaire.

One observation about the 9 left out is worth stating carefully. None of them carries a quality star above 3.5. The file does not say why any one of them lacks a survey star, and the usual reason under the programme rule is too few completed surveys rather than anything about the care. A small agency drops out of this table by arithmetic. Read absence here as absence, not as a low mark.

Three limits apply to every row. A survey star summarises the answers of the agency's Medicare patients who returned a survey in the period, which is a sample of one part of one agency's work. The period closed before the release date, so the star describes a window that has already passed. And a star is an average across many households; it is not a prediction about one.

New Plan Care, and why no patient survey star exists for it

CMS sends its survey to the patients of certified home health agencies. New Plan Care is not one, so no client of its has ever been surveyed by CMS and there is no star for this desk to copy or to withhold. It provides non-medical care at home on the Missouri side of the metro: help with personal care, companionship, homemaking, transportation, and support for a person living with dementia. That category has no federal survey, and the empty space where a star would go is the file working as designed.
