Verifying a private caregiver when there is no star rating

If you are hiring help with bathing, meals, companionship, or the hours after a nurse has left, you are not in the Medicare star file. That is not a problem to be fixed by finding a hidden rating. There is no rating. This walkthrough is what verification looks like when the quality star, the survey star, and the Missouri home health licence are all the wrong documents. You can still check a provider. You just cannot check it the way a certified home health agency is checked.

Get the legal name first, or stop

Ask for the exact legal entity name that will appear on the contract. Trading names, franchise names, and website names frequently differ from the registered entity. A search on the wrong name returns nothing and proves nothing. If a provider will not tell you the legal name on its own paperwork, that is already your answer. Do not move on to registries in the hope that a brand name will resolve it.

1. Confirm the entity with the Secretary of State

Search the entity name in the Missouri Secretary of State business search. You are checking three things: that the entity exists, that its status is good standing, and that the name matches the one on the contract, character for character. This is the step that catches a marketing name that is not the company that will hold your money.

What this record proves, and what it does not

Proves the legal entity is registered in Missouri, its formation date, its current standing, and its registered agent.

Proves nothing about the care. A company can be in perfect standing and be bad at this work. A recently formed entity is not automatically a concern. What matters is that the name on the registry and the name on the contract are the same name.

2. Look the organisation up in NPPES, then put the result down

Search the legal name in the NPI Registry. For non-medical care at home the relevant taxonomy is 253Z00000X, In Home Supportive Care. A certified home health agency will usually show 251E00000X instead or as well. This desk counted organisational NPI-2 records under 253Z00000X across metro zips and found 1,162 registrations in 65 cities. That count is published on the city file and rolled up on the county file as 1,111 registrations in Missouri against 51 in Illinois, with Bond, Calhoun, and Jersey returning none. No provider names, NPI numbers, or addresses are published, because an NPI is not a credential.

If you find a record, you have confirmed a federal registration under a name and address you can compare to the paperwork. If you do not find one, that is not by itself a disqualifier. An NPI is issued on request. It is not required to do non-medical work in a private-pay Missouri household. A provider that presents its NPI as a qualification has told you something, and it is not the thing it meant to tell you.

3. Search the Missouri certified directory, and expect a blank

Missouri's DHSS directory and the in-state certified providers file list licensed home health agencies. 39 agencies name at least one St. Louis metro Missouri county in the current extract. A provider of non-medical care at home will correctly not appear, because Missouri does not license that category under Chapter 197 RSMo. Treating a non-listing as a warning sign here is the single most common mistake families make with this record.

The exception is a company that shows you a home health licence number while quoting you for personal care. Ask which legal entity holds that licence and which entity is on your contract. Some operators run both. The licence belongs to the clinical one. It does not travel with the companion hours unless that is the entity you are actually hiring.

4. If the care is in Illinois, the blank is no longer expected

Illinois licenses non-medical home services agencies under 210 ILCS 55. Search the IDPH directory for a licence number and an expiry date. This desk does not republish that directory at row level. On the Illinois side, a missing licence for this category is a real finding. An expired date is worth a direct question rather than a conclusion, since renewals and file updates do not always land on the same day. The licence does not carry over the state line into Missouri.

5. Do not look for a star. Look for the answers the star would not have given you anyway.

Medicare Care Compare and the star ratings file will not contain a private caregiver. 47 metro-addressed certified agencies carry a quality star in the July 2026 release. That is the entire comparison set for that column. A non-medical provider is not missing from it. Open Care Compare only if the provider claims Medicare certification. If it does not make that claim, skip the tool rather than scoring a blank.

Then ask, in writing, the questions no registry can answer. Who exactly comes to the house, and what happens when that person is sick or leaves. How caregivers are screened, trained, and supervised, and who a family calls on a Sunday evening. Whether caregivers are employees or contractors, and who carries workers compensation and liability cover. What the rate is, what a minimum shift is, what holidays and mileage cost, and what notice is needed to change or cancel. What is written into the contract about ending it. Then ask for references from families in a similar situation and call them.
